Techniques of Breast Feeding

Ideally breast feeding should be started as soon as possible after delivery and it is good to start breast feeding within one hour after delivery. Sometimes there may be some problem in breast feeding at the beginning as the baby may not be able to suckle properly or the mother may not be able to nurse properly or there may be insufficient milk production.
The first milk known as colostrum is yellowish in color and thick which is packed with all the nutrients needed by the infant and also has substances that protect the infant from infections by fighting them. The colostrum is very essential for the baby. After the expression of colostrum the flow of milk in mother’s breast increases for few days and enough milk is produced for the baby’s requirement. If the mother is relaxed, the milk production normalizes within few days after delivery.
The mother should touch the center of the check of the baby or the center of the lips with the nipple which stimulates the baby to open mouth, which is known as “rooting reflex”. This is called so (rooting reflex) because this makes the baby to identify the nipple of the mother. When the baby suckles the nipple along with areola (dark brown skin which surrounds the nipples) should be inside the mouth of the baby. The lips and gums of the baby should be around the areola while suckling.
Breast feeding is not painful and if there is pain while breastfeeding; it may be due to faulty technique of breast feeding or sore nipple. If there is sore nipple it should be treated appropriately.
For the first four to six months the baby do not require any supplementation, not even water, if the breast milk is produced sufficiently is known as “exclusive breast feeding”. Doctors recommend “exclusive breast feeding” to all mothers if they are producing enough breast milk. If baby is feeding well, the baby gets sufficient quantity of water and nutrients from breast milk alone.

The appetite suppressant action of Phentermine is exerted by inhibiting the hypothalamic feeing center. The inhibition is mainly as a result of release of noradrenalin which is a neurotransmitter (chemical messenger) in brain and signals to inhibit the hypothalamic feeding center. Phentermine also have action outside central nervous system in the peripheral areas where it releases adrenaline which cause breakdown of fat cells in the adipose tissues. But the main mechanism of weight reduction of Phentermine is appetite suppressant by inhibiting hypothalamic feeding center.
How to use Phentermine for weight loss?
Phentermine, if used for weight loss by obese individuals, should be used for short term up to 3 months maximum. Phentermine should be used along with general non drug measures for weight loss like regular physical exercise, consumption of high fiber diets liberally and restriction of total calorie consumption daily for successful weight loss. Ideally if Phentermine is used for 3 months also it should be interrupted a few times in this 3 months course, e.g. give a break of up to one week after taking Phentermine for about a month. The interruption can help in reducing side effects to a large extent.
What side effects should be kept in mind while taking Phentermine?
Phentermine like any other anti obesity and weight loss drugs although relatively safe drug but it can cause many side effects. Some of the side effects are common and some are relatively less common. Some of the side effects are mild but some side effects of Phentermine can be potentially serious. Common side effects of Phentermine are nausea, vomiting, unpleasant taste in mouth and dryness of mouth, diarrhea, headache, dizziness, fatigue, libido may be less, acidity, pain in stomach, insomnia etc. Psychiatric side effects like psychosis, euphoria, nervousness, confusion, irritability etc. are also common.
Less common side effects of Phentermine are fever, weakness, fluctuating blood pressure, loss of consciousness, convulsions, hand tremors etc. Some less common psychiatric side effects of Phentermine are hallucination, aggressiveness, restlessness, panic etc. Severe diarrhea and severe vomiting can also occur as side effects of Phentermine, but are less common.

Health in perspective of global scenario or “global health” is not a discipline in itself, but it is a collection of problems (health related problems as well as economic, technical, administrative and other non health related problems). A single review of global health can only find out and lay out the major problems that by the global community and chalk out the application of modern medicine in settings of great poverty or across national boundaries.
There are many international bodies, which are engaged in tackling of major global health issues like AIDS/HIV, malaria, tuberculosis, influenza, major communicable diseases etc. to name a few. Some of the major international bodies engaged in global health problem are WHO (World Health Organization), UNICEF (United Nations Children’s Fund) and many more. These international organizations are trying to solve some basic issues in health care. But unfortunately health may be a subject of health care professionals and health care administrators; the issue of health is also an issue of social justice and social and community development. Without social justice and comprehensive social development (economic development, educational development, social development etc.) the global health problems can not be addressed successfully. Finally the issue of health, including global health is a political issue and without political will the issue of health and health related problems can not be solved or curved.
Former Prime Minister of India, late Mrs. Indira Gandhi said in one of her speech “The most important vaccine to prevent disease and illness is economic development”. And this comment was very true; without true economic development the issue of health will remain unsolved and no matter what State or individuals do to improve health in general, without economic development it will remain unsolved and as a burden. The political leadership should make economic development as their number one priority to secure a healthy environment for us and also for the future generations to come.
To solve the global issues on health, we need to solve local issues first, especially in poor and developing regions of the world. These days of “global village” era, no country (including developed industrial countries) is immune from effects of a pandemic disease, so each and every human being of this planet should work towards a goal of improved global health.

Health is actually a dynamic concept and it is not a “state”. Health should make people able to work well, live well and enjoy themselves well. The World Health Organization definition of health is considered by many authorities as being too idealistic goal rather than a realistic. Many consider the definition of health by World Health Organization as irrelevant in modern day life, as according to the definition, nobody can qualify as healthy i.e. perfect biological, psychological and social functioning. So if we accept the definition of “health” by World Health Organization, we are all sick.
